I acknowledge that both my child and I have an agreement with room2move for the ‘monkey moves’ class and that the terms of the contract include:

I am required to supervise my child and have the responsibility for my child while they are at room2move. Both my child and I hold harmless room2move
and their staff from and against any liability to either of us arising from or as a result of negligence. In no circumstance shall room2move be liable for
special or consequential damages.

Any missed classes may be made up in the term that I am currently enrolled in. No refunds will be given for missed classes.

My personal information will be kept confidential.

I understand that room2move and the monkey moves staff will not be able to provide me with their services without this consent.
